if you're in hokkaido, noboribetsu onsen near sapporo is the best i've been to, hands down. there are about 7 indoor and 4 outdoor baths. the indoor vary in mineral content and temperature, and are laid out in this huge room that sort of makes you feel like roman royalty. and the outdoor baths are really picturesque, especially when the snow is falling on the pines. oh, and the shampoo/rinse/body soaps are amazing and sell on sephora.com for around $40 apiece.
in kansai, kinosaki onsen has some lovely baths and is just a nice town in general for a day trip.
i also just spent a weekend in hakone and while i didn't go to any onsen besides the one at our hotel, there are a ton of public ones as well.
I've always been a fan of the one in Kurama. Take the hike up there, get all sweaty and gross, relax in the onsen, then take the public transit back down to Kyoto. A nice day trip.
Naruko in Miyagi is also really nice. They have a lot of little onsens, some with outdoor baths, some with private tubs, etc. Even the ryokan we stayed in had a natural hot spring in its basement. http://www.jnto.go.jp/tourism/en/16.html
